Insurance Verification
Before your first appointment, our office can help review:

Verification of benefits does not guarantee payment. Final coverage and reimbursement decisions are determined by your insurance plan.
Out-of-Network Options
If your insurance plan is not accepted, you may still choose to receive care through our self-pay option.
A superbill can be provided upon request for possible reimbursement through out-of-network benefits. Reimbursement is not guaranteed and depends on your individual insurance plan.
Straightforward Pricing
Self-pay appointments are available for patients without insurance or those who prefer to pay directly.

Payment is due at the time of service. Please contact our office to confirm current rates and payment options.
Cancellation & No-Show Policy
Your appointment time is reserved specifically for you. Please provide at least 24 hours' notice if you need to cancel or reschedule.
Appointments canceled with less than 24 hours' notice or missed without notification may be subject to a $50 fee, where permitted by applicable law and insurance agreements.
Exceptions may be considered for emergencies or extenuating circumstances, consistent with practice policy.
